DOCUMENTS TO PROVIDE :

Attention since July 2007 the City Council whatever the statement affixed must indicate on each document the name and the quality of the signatory

Accreditation / Social survey / Psychological survey	You must send us either the originals or certified copies by the general council or the town hall of your domicile. If the general council has only provided you with copies, we send these documents without going through the town hall - copies of copies are prohibited As a general rule, we do not legalize the notice which must imperatively be presented to the Ministry nevertheless. If you wish to have this document legalized, please note it in observation on the order form	160 € per document
The birth certificates of each person in the household And, where applicable, the marriage certificate	Send us original excerpts. Copies of the family booklet are not accepted.	160 €
Application (letter from adopters)	Send us the originals that you have previously made legalize by your City Council. The City Council must note the following: "Seen for physical certification of the signature of ..." Each signature must be legalized.	160 €
Medical certificates for each person in the home	To send us the originals which must be legalized beforehand by the national council of the order of doctors or by the town hall which must in this case legalize the signature of the practitioner.	160 €
The criminal record extract	(One per adult) Send us the original dating of less than 6 months	160 €
Pay slips for each person in the home	Make a copy certified by the City Council	160 €

All entries and stamps must appear on a printed page. A certification on the back of a document, if it is blank, is refused by the Ministry.

Do not forget to indicate to us on the enclosed order form your address and a telephone number to allow us to reach you in case of problem.

If the amount of the check is not correct we do not block the file. We claim the difference or refund the overpayment.
