DOCUMENTS TO PROVIDE

Attention since July 2007 the City Council whatever the statement affixed must indicate on each document the name and the quality of the signatory
Each document must be translated into English by a sworn translator who must have his or her signature signed by the City Council or the Chamber of Commerce
Send us the copy in French plus the copy in English .
Both copies must be legalized (we can take care of translations if you wish)

General Candidate Information Form	Send us the original that you have previously made legalized by your City Council. The City Council must note the following: "Seen for physical certification of the signature of ..." Each signature must be legalized.	37 € x 2
Medical certificates for each person in the home	To send us the originals which must be legalized beforehand by the national council of the order of doctors or by the town hall which must in this case legalize the signature of the practitioner.	37 € x 2
The criminal record extract	(One per adult) Send us the original dating of less than 6 months	37 € x 2
Formal application for adoption	Send us the original. Have all signatures legalized by the City Council	37 € x 2
Work certificates	Make copies and have them certified by the town hall of your home The town hall shall affix the following words: "certified true copy" Unless your employer and the persons who have sent you the letters have already had their signatures authenticated by their respective town halls - in this case we send the originals	37 € x 2

Note: City halls are no longer allowed to certify copies of documents as a general rule. This rule is not applicable for documents intended to be produced abroad and you can insist if necessary.

All entries and stamps must appear on a printed page. A certification on the back of a document, if it is blank, is refused by the Ministry.
