DOCUMENTS TO PROVIDE

Attention since July 2007 the City Council, whatever the indication affixed, must indicate on each document the name and the quality of the signatory
All documents must be provided in original (except civil status documents) certified copies are no longer accepted by the adoption center
All entries and stamps must appear on a printed page. A certification on the back of a document, if it is blank, is refused by the Ministry

Accreditation - the social survey - the psychological survey	You must send us the originals issued by the General Council As a general rule, we do not legalize the notice which must imperatively be presented to the Ministry nevertheless. If you wish to have this document legalized, please note it in observation on the order form Attach two additional single copies of the accreditation for our archives
Full copy of the marriage certificate	Document to claim to the Mary having celebrated the marriage
Application for registration of the application file to the Adoption Center of the Ukrainian Ministry of Education, together with the list of documents	Send us the original that you have previously made legalized by your City Council. The City Council must note the following: "Seen for physical certification of the signature of ..." Each signature must be legalized.
Medical certificates for each person in the home	To send us the originals which must be legalized beforehand by the national council of the order of doctors or by the town hall which must in this case legalize the signature of the practitioner.
Criminal record extract n ° 3	(One per adult) Send us the original dating of less than 6 months
Certificate of immigration and attestation concerning the conditions of recognition in France issued by the SAI
Last imposition opinion	Ask for a duplicate at your tax center, sign it and have your signatures legalized by the City council which must affix the mention "seen for physical certification of the signature of ..."
Possible power of attorney	Established before the notary who must affix the mention "seen for physical certification of the signatures of ... .." and affix his Marianne (the commercial stamp is not enough)
Commitment on Honor	Have your signatures legalized by the City Council
Attestation of the child's follow-up by ESA	Have the signatures legalized by the City Council

COST OF LEGALIZATION / TRANSLATION	Insofar as the prices of the translations vary, we will contact each family to communicate to them the costs practiced by each organism after having taken the steps
